How do you keep the insulation in place in the sub-floor?

A good method is to measure down the height of the insulation and fix a few nails in the joists so that the insulation cannot fall through.

How do you fix the walls to stop them falling down ?

When putting them up fix a temporary cross brace (a diagonal timber) until you have all 4 up and they can support themselves.

Why does the insulation go under the concrete?

It helps with the thermal mass of the concrete. The idea being that due to the density of the concrete, it will absorb heat so by insulating under it, it helps the concrete to retain the heat, thus retaining heat in the floor/room.

I thought EPS insulation should be used for a load bearing slab and not PIR?

I used PIR, but you can use either. The thermal efficiency of PIR is higher so the material is more expensive than EPS.

How do you fix the walls to the floor?

The walls are screwed into the floor using 5mmx100mm turbo gold screws.

Will frost heaving be an issue?
